A care service providing help to people in their homes has been judged as requiring improvement after concerns were raised.
Inspectors who visited House of the Flame Lily, in Penn Road, Wolverhampton, found the service was in breach of five legal regulations, a report published on Wednesday, external said.
The Care Quality Commission (CQC) report said risks were not always fully considered and there were concerns over safety at the service, which provides home care to people of all ages with disabilities. However safeguarding procedures were followed.
Registered manager of the service Fungai Ndemera said the service had submitted an action plan to the CQC and addressed all concerns.
'No harm found'
Inspectors who visited the agency between 31 October and 7 November found the service, which was looking after seven people, was in breach of several regulations including safe care and treatment.
"Risks to people were not always fully considered based on their assessed needs and we found concerns with the safety of people's environment," the report said.

Registered manager Fungai Ndemera said staff had addressed all concerns
Medicines were not always managed safely and inspectors found staff had not always received training relevant to people's needs.
Systems for monitoring and improving the service were not effective, and audits had not identified concerns, inspectors said.
However, the report found safeguarding procedures were followed and there was a system in place to reduce risk of cross-infection.
Staff had received mandatory training and were happy working for the company and felt supported, the report said.
After the report was published, Mrs Ndemera said: "We have taken active and comprehensive action already.
"The CQC confirmed there was no harm to any person using our service.
"Issues raised were primarily related to documentation."
Mrs Ndemera added: "We have implemented all action plans and refreshed training, risk assessments and governance systems.
"We are working closely with the CQC to that effect."
